If you're one of the more than 26 million people in the U.S. with diabetes, you know first hand how the disease impacts your life. That includes your dental health — and whether or not implants are a good tooth replacement option for you.
Diabetes is actually the name for a group of diseases affecting how your body processes glucose, a simple sugar that provides energy for the body's cells. The level of glucose in the blood is regulated by insulin, a hormone produced in the pancreas. Diabetes causes the pancreas to either stop producing insulin (Type 1) or not produce enough (Type 2). Also in Type 2, the body can become unresponsive to the insulin produced.
The implications for either type are serious and can be life-threatening. If glucose levels are chronically too low or high the patient could eventually go blind, suffer nerve damage, or develop kidney disease. Diabetes also interferes with wound healing and creates a greater susceptibility for gangrene: diabetics thus have a higher risk for losing fingers, toes and limbs, and can even succumb to coma or death.
Type 2 is the most common form of diabetes. Fortunately, most people with this type can effectively manage it through diet, exercise and regular glucose monitoring; if need be, prescription medication can help regulate their levels. Even so, diabetics with their disease under control must still be alert to slower wound healing and a higher risk of infection.
Because implant placement is a minor surgical procedure, the aspects of diabetes related to healing, infection and inflammation could have an adverse impact on the ultimate success of the placement. Implant surgery creates a wound in the surrounding gum tissues and bone that will need to heal; the body's immune response in a diabetic can interfere with that process. And if infection sets in, the risks of implant failure increase.
But research has shown that diabetics with good glucose management have as high a success rate (over 95% after ten years) as non-diabetic patients. That means the implant option is a viable one for you as a diabetic — but only if you have your disease under control.
In the winter months, when the daylight hours grow few, we celebrate the season with holiday lights and good cheer. This season of hope and renewal also gives us a chance to think about the future — to take stock of where we’ve been, and even plan for some changes in the new year.

Besides being an obvious blemish on your appearance, missing teeth also create problems you can’t see. For one, it’s harder to eat a proper, balanced diet if you have trouble chewing certain foods, potentially leading to serious nutritional problems. For another, when teeth are lost, the bone in your jaw that used to surround them begins to deteriorate. This can cause you to have a prematurely aged look, and make you appear unhappy even when you’re not.
Unfortunately, dentures don’t solve these problems — in fact, they tend to compound them. Many denture wearers report they have problems eating; some even say they eat better without dentures. And dentures don’t stop bone loss; in many cases, especially when they don’t fit correctly, they actually accelerate it.
But there’s really no reason you have to get by with missing teeth. Since they were introduced some three decades ago, dental implants have offered people a better way to replace lost teeth. With implants, you can eat your favorite foods again, smile with complete assurance, and stop worrying about dentures that may fit poorly or slip out at the wrong times.
Fixed solidly in your jaw in a minor surgical procedure, dental implants function just like your natural teeth. Their natural look and “feel” makes it easy to forget they aren’t the teeth you were born with. Best of all, they can last the rest of your life… unlike bridges or dentures. Because they offer a permanent solution, implants can be quite cost-effective in the long run. But the way they can restore your confidence and make you feel good about yourself isn’t something you can put a price on.

The dental implant is the closest thing in modern dentistry to a natural tooth. This is because an implant replaces more than the visible crown — it also replaces the root, thanks to a metal post imbedded in the bone.
But what if you have a metal allergy — are you out of luck replacing a tooth with an implant? Before answering this question, let's take a closer look at metal allergies.
An allergy is an overreaction of the body's immune system to a particular foreign substance. This response can be as inconsequential as a minor rash or as life-threatening as a shutdown of the body's organ systems. You can be allergic to anything, including metals.
Usually, these allergies are to specific kinds of metals. For example, about 17% of women and 3% of men are allergic to nickel, while smaller percentages are allergic to cobalt or chromium. Most allergic reactions to metal occur from external contact with jewelry or similar metal items that create rashes or other anomalies on the skin. On a more serious note, an allergy to metal in a body replacement part could result in the body rejecting it.
Metals have also played an important role in dental care, particularly dental amalgam used for tooth fillings. Dental amalgam is a mixture of a precious metal like gold or silver with other metals like copper, tin and, in small amounts, mercury. While dental amalgam has been used safely for decades, there have been rare cases of inflammation or rashes.
This brings us to dental implants and the most common metal used in them, titanium. The commercial version of this metal is highly prized in medical and dental applications because it has a special affinity with bone. Bone cells readily grow and adhere to the metal, which strengthens the bond between the implant and the jawbone.
Even if you have a rare allergy to certain metals, it's even rarer that would include titanium. In one particular study of 1,500 implant patients less than 1% reported any reaction at all.
If you're concerned, you can undergo testing to see if you react to titanium. More than likely, though, you'll be able to join the millions of other patients who have successfully restored their smiles with dental implants.

People have depended on dentures for generations—and they still do. That's because they work, both in restoring dental function and a smile marred by missing teeth.
But they have one major drawback related to bone health. That's because living bone has a life cycle: as older cells die, new ones form to take their place. The pressure generated when we chew stimulates this growth. But when this stimulus goes missing along with the teeth, the cell replacement rate slows and bone volume and density gradually diminishes.
Traditional dentures can't transmit this chewing pressure stimulus. And because they rest directly on the gum ridges, they can adversely affect the underlying bone and actually accelerate bone loss.
But implant technology potentially solves this bone loss problem with dentures by using implants rather than the gums to support them. It's a two-fold benefit: first, the implants relieve much of the irritation to the gums and bone caused by traditional dentures. Primarily, though, the implants themselves can slow or even stop continuing bone loss.
Most implants are made of titanium, not only because it's compatible with the body, but also because it has an affinity with bone. Over time bone cells grow on the titanium post imbedded in the jawbone. This process not only creates stability and durability, it can improve bone health.
In recent years dentists have incorporated implants with dentures to create two exciting treatment options. With one option, the dentist installs two or more implants in the jaw, to which a specially fitted removable denture can be attached. You would still have the ease of removing the denture for cleaning, while gaining greater stability and a reduced risk of bone loss.
The other option is a fixed denture (or bridge) attached permanently to implants. For this option, a patient's jawbone must be adequate and healthy enough to support at least four to six implants. A fixed denture is also often costlier and more complex than a removable denture, but it can feel more like real teeth. It also promotes better bone health too.
Although both options are more expensive than traditional dentures, they can pay dividends for long-term dental health. Implants could help you enjoy your new dentures and resulting smile for a long time to come.

Waiting is part of life for a teenager: waiting to get a driver’s license, to graduate high school or to leave home and stretch their wings. A teenager with lost teeth may also need to wait until they’re older to obtain dental implants.
The reason arises from the differences in how implants and natural teeth attach to the jaw. Although natural teeth may seem rigidly set in the bone, they’re actually held in place by an elastic tissue between them and the bone known as the periodontal ligament. Tiny filaments that attach to the teeth on one side and the bone on the other hold the teeth in place, but also allow the teeth to move gradually in response to mouth changes.
A titanium implant post doesn’t have this relationship with the periodontal ligament — it’s attached directly to the jaw bone. Over time the bone, which has a special affinity with titanium, grows and adheres to it to form a durable bond without an attachment to the periodontal ligament. Because of this the implant can’t move like a natural tooth.
This is extremely important for implant placement because the jaws in particular won’t fully develop in most people until their late teens or early twenties: the upper jaw in particular will tend to grow out and down. Natural teeth accommodate to these changes, but the implant can’t — it will appear to retreat into the jaw. The gum tissues surrounding the implant also won’t conform to the continuing growth and may appear receded.
The best approach is to choose a temporary replacement option until the jaws and other facial bone structures have finished growing. One example is a bonded bridge in which we use a bonding agent to attach a bridge of artificial teeth to teeth on either side of a missing tooth — bonding won’t permanently alter them as with a traditional bridge. Once the jaws have finished growing, we can remove the bonded bridge and install the more permanent implant.
Ask any teenager: waiting can be hard. But with dental implants, waiting until the right time will help ensure the attractive result is a permanent one.
